A medication is a chemical product, and like any chemical, the molecules it contains can change over time as they are exposed to their surroundings. How medications change over time

The active and inactive ingredients in a medication are molecules that can slowly undergo chemical reactions. Three pathways are studied across pharmaceutical chemistry: hydrolysis, in which a molecule reacts with water; oxidation, in which it reacts with oxygen; and photodegradation, in which light supplies the energy that drives a molecule to break down. These reactions are generally understood to proceed faster as temperature rises.

Because these processes are gradual rather than sudden, storage guidance is designed to slow them, not to stop them completely. That is why every preparation is assigned a dated limit — an expiration date for a manufactured drug, or a beyond-use date for a compounded preparation — after which it is no longer expected to remain within its intended specifications. Good storage is what those dates assume.

Light, heat, and moisture

Most storage instructions come down to limiting a preparation's exposure to the three conditions understood to speed the reactions above. In broad terms:

  • Heat — chemical reactions generally proceed faster at higher temperatures, which is why many products specify a cool storage range and some require refrigeration. Spots like a bathroom cabinet or a parked car can swing well outside a typical room-temperature range.
  • Light — some molecules are photosensitive, so they are often packaged in amber vials, opaque containers, or a carton that shields them from direct light.
  • Moisture — humidity can drive hydrolysis and affect solid forms such as capsules and tablets, which is why containers are kept closed and dry and sometimes include a desiccant.

Keeping a medication in a cool, dry place away from direct light is the general principle behind most labels. The specific range that applies to your preparation, though, is the one printed on its container.

Temperature ranges and the cold chain

Pharmacy storage instructions usually reference standard categories rather than a single number. "Controlled room temperature" describes a defined, moderate range, while "refrigerated" describes a colder range used for products that are more temperature-sensitive. Each term maps to a specific range in pharmacy practice, and the label tells you which one applies.

Refrigerated preparations, which include many injectables, are handled through what is often called a cold chain — an unbroken sequence of temperature-controlled steps from the pharmacy to your door. Freezing is a separate consideration: for some formulations, freezing is understood to cause physical changes, so a number of refrigerated products are labeled specifically to avoid it. When a temperature-sensitive shipment arrives, following any instruction on the label — for example, refrigerating the preparation promptly — helps keep it in the condition the pharmacy released it. How a medication's form shapes its storage

The physical form of a preparation influences how it is stored. Solid oral forms such as capsules and tablets are generally most sensitive to moisture; creams and other topicals have their own container and temperature considerations; and oral liquids and sterile injectables are frequently the most temperature-sensitive, which is why many are refrigerated.

Whether a preparation is sterile or non-sterile also matters. Sterile products, including injectables, are prepared and packaged under stricter standards, and their storage instructions reflect the need to keep them within a narrow range. Some preparations also change category once they are opened, mixed, or reconstituted — the beyond-use date and storage conditions after that point can differ from those of the sealed container.
