Telehealth Informed Consent

This is the canonical informed-consent document presented during the OpenDoseRx medical intake. Before any prescription product request can be reviewed by a licensed provider, you must read and accept the clauses below. The version identifier above is recorded with your acceptance.

Consent Clauses

  1. Nature of Telehealth Services.

    OpenDoseRx provides a technology platform through which you can request prescription products and complete a structured medical intake. Clinical evaluation is performed remotely by independent, licensed healthcare providers using the information you submit. Telehealth involves the delivery of healthcare services without an in-person visit, using electronic communications to transmit your health information to a provider.
  2. Asynchronous Review.

    Your intake may be reviewed asynchronously, meaning a licensed provider evaluates your submitted information at a later time rather than during a live audio or video visit. You may not interact with the provider in real time. The provider may contact you with follow-up questions, request additional information, or require a synchronous (live) consultation before making any clinical decision.
  3. Provider Discretion; Provider May Decline.

    All clinical decisions, including whether any prescription is appropriate, are made solely by the reviewing licensed provider in their independent professional judgment. The provider may decline to issue a prescription, recommend an alternative, or direct you to in-person care. OpenDoseRx does not practice medicine and does not influence clinical decisions.
  4. Risks and Limitations of Telehealth.

    Telehealth has inherent limitations. The provider does not perform a physical examination and relies on the accuracy and completeness of the information you provide. Possible risks include, without limitation: information transmitted may be insufficient to allow appropriate decision-making; delays in evaluation may occur due to technology failures; and, in rare circumstances, security protocols could fail, causing a breach of privacy of personal health information. Incomplete or inaccurate answers in your intake may result in inappropriate care recommendations.
  5. Not for Emergencies.

    OpenDoseRx and its affiliated providers do not provide emergency services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this platform to seek care for urgent or emergency conditions.
  6. Privacy and Security.

    Information you submit, including health intake responses, is handled in accordance with the OpenDoseRx Privacy Policy and applicable law. Your information is shared with the licensed provider reviewing your request and with pharmacy and fulfillment partners as necessary to deliver the services. Reasonable administrative, technical, and physical safeguards are used to protect your information.
  7. State Licensure and Availability.

    Telehealth services are provided only by providers licensed in the state where you are located at the time of service. Service availability varies by state, and certain products or services may not be available in your state. You agree to provide accurate information about your location.
  8. Right to Withdraw Consent.

    Your participation is voluntary. You may withhold or withdraw this consent at any time before a clinical decision has been made by contacting support, without affecting your right to future care or treatment. Withdrawal of consent may mean that the services cannot be provided to you.
  9. No Guarantee of Prescription or Outcome.

    Submitting an intake or completing a purchase flow does not guarantee that a prescription will be issued or that any particular clinical outcome will result. No statements on this platform should be interpreted as a promise of treatment, results, or benefits. Charges associated with declined requests are handled under the Refund Policy.
  10. Consent Version and Effective Date.

    This consent is version "consent-v1", effective June 12, 2026. By checking the consent box during intake, you acknowledge that you have read and understood this document, that your questions have been answered to your satisfaction, and that you consent to receive services via telehealth under these terms. The version you accept is recorded with your intake.

Acknowledgment

Acceptance is recorded electronically when you check the consent box during intake, together with the consent version and a timestamp. If this document is revised, a new version identifier will be issued and you will be asked to accept the updated terms before your next request.
